Requirements for Obtaining a Maywood Public Library Card
If the photo ID does not have a current address, 2 additional forms of ID are needed for proof of residency, such as an auto insurance card, medical bill, property tax bill, auto registration card, medical card, check book, utility bill, hospital bill, or a voter's registration card. High School students under the age of 18 must show a School ID and a Drivers Permit. The student's parent or legal guardian must be present to sign the application, and the parent or legal guardian must show 2 ID's. One of the IDs must be a Drivers License or Illinois State ID.
